Well-Child Visit and Vaccine Schedule

(At birth, your baby should have received a Hepatitis B immunization, hearing test, and newborn screen)

Doctors visits by age:


5 Days:Hospital Follow-Up and Weight Check
2 Weeks:Check-Up and Repeat Newborn Screen
1 Month:Weight Check
2 Months:Check-Up and Immunizations (3 shots and 1 by mouth)
4 Months:Check-Up and Immunizations (3 shots and 1 by mouth)
6 Months:Check-Up and Immunizations (3 shots and 1 by mouth)
9 Months:Check-Up, Developmental Screening, Labs (hemoglobin)
12 Months:Check-Up and Immunizations (4 shots), Labs (lead level)
15 Months:Check-Up and Immunizations (2 shots), Dental Flouride
18 Months:Check-Up, Developmental & Autism Screening, Immunization (1 shot), Dental Flouride
2 Years:Check-Up, Developmental & Autism Screening, Labs (hemoglobin & lead level), Dental Flouride
2 ½ Years:Check-Up, Developmental Screening, Dental Flouride
3 Years:Check-Up, Dental Flouride
4 Years:Check-Up, Developmental Screening, and Immunizations (4 shots)
5 to 10 Yearly: Check-Up (with additional labs and catch-up immunizations as needed)
11 Years:Check-Up and Immunizations (2 or 3 shots)
12 and Up Yearly: Check-Up (additional labs and catch-up immunizations as needed)

We recommend that all children over age 6 months receive a Flu Shot in Fall-Winter Yearly

Can give FluMist nasal vaccine if over 2 Years
If 1st time receiving flu shot then they need a 2nd dose in a month

Appointment Policy

We want to provide your child with the best possible care and will always try to be accommodating but please read the following to help us help you.

Sick or Same Day Visits are available 5 days a week. Appointments are usually on a first-available basis.
A staff member will call you 1 or 2 days before an appointment as a reminder. Make sure we have a good phone number at all times.
Call if you are running late or if you will not be able to make your appointment. Late arrivals (> 30 minutes after their scheduled time) will lose their time slot and be seen on a first-available basis. Walk-ins will also be seen on a first available appointment time basis.
Arrive early if possible. Remember to go by Patient Accounts prior to each visit.
Appointments for additional children should be made by phone before coming to the office.
Please bring at least one adult to accompany every two children in the exam room However, no more than two adults per child will be allowed into the exam room.
Turn off cell phones in the office and exam rooms.
Missed Appointments:
- After the first missed appointment, we will contact you by telephone to obtain an explanation. The appointment may be rescheduled. If a parent or guardian is not reached, then a letter will be mailed notifying you of our office policy and the missed appointment.
- After the second and third missed appointments, we will contact you by telephone and a letter will be mailed notifying you of our office policy and the missed appointment. The appointment may be rescheduled. Your case manager will be notified of the missed appointments as well.
- After the fourth missed appointment in one year, it may be necessary to terminate our professional relationship with the patient and family. A certified letter will be sent. The appointment may not be rescheduled.
- We understand that extenuating circumstances do occur and will take this into consideration when an appointment is missed.

Vaccine Policy Statement

We firmly believe in the safety of our vaccines.
We firmly believe in the effectiveness of vaccines to prevent serious illness and to save lives.

We firmly believe that all children and young adults should receive all of the recommended vaccines according to the schedule published by the Centers for Disease Control and the American Academy of Pediatrics.

We firmly believe, based on all available literature, evidence and current studies, that vaccines do not cause autism or other developmental disabilities. We firmly believe that thimerosal, a preservative that has been in vaccines for decades and remains in some vaccines, does not cause autism or other developmental disabilities. The recommended vaccines and their schedule given are the results of years and years of scientific study and data gathering on millions of children by thousands of our brightest scientists and physicians. Because of vaccines, many of you have never seen a child with polio, tetanus, whooping cough, bacterial meningitis or even chickenpox, or known a friend or family member whose child died of one of these diseases.

Should you have doubts, please discuss these with your doctor. Please be advised, however, that delaying or breaking up the vaccines to give one or two at a time over two or more visits goes against expert recommendations, and can put your child at risk for serious illness (or even death) and goes against our medical advice.

If you should absolutely refuse to vaccinate your child despite all our efforts, we will ask you to find another health care provider who shares your views. We do not keep a list of such providers nor would we recommend any such physician. Please recognize that by not vaccinating you are putting your child at unnecessary risk for life-threatening illness and disability, and even death.

As medical professionals, we feel very strongly that vaccinating children on schedule with currently available vaccines is absolutely the right thing to do for all children and young adults.
